Collegial Covenant Groups Project
Dear Collegial Covenant Group Facilitator:
As a part of the evaluation process for the Collegial Covenant Groups Project, we are asking facilitators and members of groups to complete questionnaires and return to the Ministers Council office before the end of August. As the facilitator, we are asking you to administer the questionnaires to your group members following the procedures outlined below:
Please set aside about 15 minutes during your next group meeting to have the members complete the questionnaires.
Please photocopy the Covenant Group Member’s instrument in sufficient quantity for all your group participants.
To assure that the confidentiality of the members is protected in this process, please provide an envelope of appropriate size for the number of members in your group. We ask that you pass this envelope around and have each member deposit his/her own completed questionnaire in the envelope.
Please do not read any of the responses provided by your members.
Please complete your questionnaire. Note that the facilitator’s questionnaire is distinguished from the others by title and by longer length.
Place your completed questionnaire in the envelope with the others and seal it.
The sealed envelope with your questionnaire and those completed by your group members should be sent to us as follows:

As the facilitator, you are asked to complete an additional set of questions. The members’ questionnaire has two parts. Your questionnaire has three parts. It is very important that you respond to each question.
Your responses are very important to us as we move continue our assessment of this project. We are collecting this information from all collegial covenant groups that were funded for this program year. The results of the surveys will be analyzed in aggregate form so that no comments will be attributed to a specific individual.
