Collegial Covenant Groups Project

 

Dear Collegial Covenant Group Member:

The attached questionnaire is being provided for you to complete as a part of our evaluation process for the Collegial Covenant Groups Project. 

It is very important that you respond to each question and place your completed questionnaire in the envelope that will be passed around by your group leader.  We have asked the leader to request that each member places his/her own questionnaire in the envelope so that all responses can remain confidential.  The leader has also been instructed to seal the envelope and return it to the Ministers Council without reading any of the responses. 

Your responses are very important to us as we continue with our assessment of this project.   We are collecting this information from all collegial covenant groups that were funded for this program year.  The results of the surveys will be analyzed in aggregate form so that no comments will be attributed to a specific individual.
