Open the form online at the Ministers Council website. Before typing any information into the form you must first save the form to your computer (file - save as). You can save it with the same file name, or rename it, but keep the "PDF" extension since this is a pdf form. The posted form online is "Read Only" and once you close the form without printing it first, all of your entries will be LOST. Once the form is saved to your computer, you will be able to open and close the form to work on it as time permits and all of your entries will be retained.
When you have completed your form, you can then save, close, and email it to the "Point Person" of your group, or the person designated to collect all of the evaluations. Once that person has received evaluations from each group member, please "SNAIL MAIL" them to the Ministers Council office.
